$30 Sports Physicals

$30 flat fee covers exam and review of sports physical form.
Parent and athlete are responsible for completing their portion of the form and bringing it to the visit.
Fee will be collected at the beginning of the visit
Sports physical visit does not include immunizations, prescription refills, or other medical care. If additional medical care is needed, parent will need to schedule a well child visit or a sick visit.
Payment of fee does not guarantee clearance if medical clearance for athletic participation is not medically appropriate.
If the parent would like the child to be seen for a traditional well child exam including immunizations, the sports physical form can be completed at that time without payment of the $30 if the child's insurance covers the visit at 100%
The $30 fee covers the cost of the sports physical and no charges will be submitted to the insurance company.

As mentioned above, the parent and athlete are responsible for providing the appropriate sports physical form as requested by the athlete's school or sports team. The most commonly used form can be found here. However, North San Antonio Family Medicine cannot guarantee that this is the form required by the athlete's team.
